Application Instructions

Welcome! to the Wave Express contractor application website. You will be using this site to complete, submit, and track your contractor application. Please utilize either a laptop or desktop computer in order to successfully complete and submit your application. Before you begin, please read the following instructions carefully.

It will take you approximately 20 to 30 minutes to complete your contractor application. You may partially complete the application, save it then log back in to finish it later. You can return to your partially completed application as many times as needed to add or edit information, then submit it. Once your application is submitted, it cannot be modified. The following items are required to submit your application for processing:

We recommend you gather the required information listed below before you begin completing your application:​

  1. Your personal information to include Current Residence Street Address, Home Phone Number, Cell Phone Number, Social Security Number, and Date of Birth.
  2. Your previous residence addresses for the past 3 years.
  3. Emergency Contact Information – list at least two people containing their Name, Contact Phone Number, and your relationship with them.
  4. Your Driver License information to include the License Number, the State it is issued in, License Type, and the Expiration Date. Please include the same information for driver licenses you have possessed in the past 3 years. We also require information regarding any suspension, revocation, or denial of a driver license at any time.
  5. Your Driving Experience. Please list all your past and present driving experience regarding Class of Equipment, Type of Equipment, and approximate driving and towing miles operated with each piece of equipment.
  6. Your Vehicle Information - Year, Make, Model, Description, VIN and the GVWR (Gross Vehicle Weight Rating) found on the manufacturers label on the inside or outside driver door jamb. Trucks must be a 3/4 ton or 1 ton, year 2005 or newer, no exceptions.
  7. Work History - To comply with DOT regulations, please list your work history for the past 10 years, do not leave any fields blank. You must also list your activity history between jobs if you have been unemployed, retired, in school, disabled, or not working due to medical reasons during the last 10 years. There must not be any gaps in dates in the last 10 years. Enter your work and/or activity history information beginning with the most recent date(s).

Important:The most common cause for delayed application processing is missing work and/or activity history information.
